All Medicare- and Medicaid-certified nursing homes that were part of the July 2023 CMS nursing home provider census made available in July 2023 were evaluated by U.S. News. Nursing homes did not apply nor did they provide any data or materials to U.S. News.

To be eligible for a Short-Term Rehabilitation, Long-Term Care or Overall rating, a home must have met both of the following inclusion criteria:

  • Received reimbursement from CMS in July of 2023.
  • Had sufficient data to evaluate quality in that rating.

  • 1,918 homes were High Performing in short-term rehabilitation.
  • 1,353 homes were High Performing in long-term care.
  • 467 were High Performing in both.

For my husband's stay, 2 wks. I don't have everyone's name but from Terri, Bonnie, Cathe, Wendy, Ashley (PT), Ed, OT guy, I think Christine, to a hand full of nurses I think his stay was great. The only issue I had was with Linda, I think she's lead nurse or something like that. Day one, without gloves on, removed bandage from my husband's foot that he had a toe amputated, touched his buttocks, checking for pressure ulcers, bare handed. Not knowing what germs she's carrying from one patient to another and him getting IV antibiotics because of a blood infection already, so unacceptable. I also asked her to initial an order from surgeon I brought in, to remove staples from his amputation, no big deal, but you would have thought I had asked her to borrow money from her. She threw her head back and rolled her eyes. No lie. She'd probably deny doing that. She was my only concern. I'm happy I was able to bring our dog in to visit and that my husband was able to go outside in courtyard an get fresh air. Other than my one issue I would recommend OCNR.

They treated me well! Had a lot of fun in physical, and occupation therapy. Activities Department is outstanding! Lots to do always! They really get you ready to safely return home.
