Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Specialty Programs

From the cheerful and efficient office staff at the front desk to the incredibly dedicated nursing team, Orange City Nursing and Rehab provided a welcoming and supportive environment for my mother. The front office staff were always helpful and accommodating, answering my questions with patience and a genuine willingness to assist. They made navigating the administrative side of things a breeze, allowing me to focus solely on my mother's well-being.
But the true heart of Orange City Nursing and Rehab lies within its nursing staff. They are, simply put, the best. Their professionalism was evident in every interaction, from administering medication with precision to ensuring my mother was comfortable and well-cared for. They took the time to truly get to know her, understanding her needs, and treating her with the utmost respect and dignity.

[review] => Within the last year my mother was there two times as a rehab patient and remained eight months as a long term care resident until her death. My thanks goes out to the helpful staff among who stood out were: the CNAs Crystal and Matthew. I could tell when either one was assigned to mom because the bed was just changed and or mom would be up in the wheelchair 😊 Also there were Charlotte, Bri, Shakira, Donna, Rosie and nurses Sage and Alex among others. Overall it was a good experience. These situations were a loved one find themselves in is hardly ever an ideal one. For what it’s worth, I’m glad mom was there. For those contemplating long term care, these decisions aren’t easy. Remember to advocate for your loved one.

The only positive thing I witnessed that was good was the aides. When you ask to see the nurse on duty and was told that they didn't have time for you, there you go. Also witnessed their physical therapy session that they charged the VA for, absolute joke. I could go on and on. I wouldn't recommend this facility at all. RIP Dad.

[review] => Went to visit a family friend who had back surgery and was saddened to see how the place is ran. It took over 20 minutes for someone to answer a call button. I ended up helping a resident across the hall from our friend because he could not move his table and had no call button for help. I broke a plate while helping the resident and went to the nurses station to let them know and they laughed. Took another 20 minutes for someone from housekeeping to come innand clean the glass. Our friend has his wife there to advocate for him. Hope he goes home soon.

[review] => My mother whom has dementia was there with a leg wound. Thank goodness I visited often as she needed an advocate.
Everyone whom works at ORC is very nice, except for one nurse whom took his sweet time to help my mom. The supervisors do not return phone calls. I was told I could not assist my mom to go to the bathroom. On one occasion I hit the call button as she had to poop. It took 30 minutes to get a CNA who said they were too busy to help her. She pooped in her pants & had to wait an hour for help. There are not enough nurses or CNA 's per patient to handle the work load. If you are going there for rehab, you may as well be on your own.
The food was pretty much frozen, canned or a sandwich... Not impressed.
That leg wound my mom was there because of... her bandage was supposed to be changed daily. She went 3 days one week where they they did not change her bandage & I brought it up to two separate nurses on two consecutive days. The following week her bandage was not changed for 2 days. I took her to ER when she was discharged on day 20 to discover her leg was infected & she had to have surgery to save her leg.
My mom got a shower, maybe twice when she was there. After 2 weeks I told a CNA she stunk & needed a shower. No one could tell me exactly when her shower days were when I asked.
She had a rash when she went in. I questioned it and was told it must be a reaction to her medication. After a dermatologist visit & Infectious Disease Dr we discovered she had scabies.. it would have been nice for this to be diagnosed sooner. It got worse in the rehab facility because it was so hot.
Her clothes were labeled with her name & a sign on her cabinet door that family does her laundry. 3 pair of pants went missing & the supervisor didn't care. One CNA went to the laundry looking when mom was discharged but that was the extent of it.
If you can, do not put your loved one here. Bring your loved one home & hire a service or independent CNA to come into your home. They will get much better care. Most insurance companies deliver a 2 week supply of meals when released from the hospital to your home. They also cover In home visits for PT, OT & nurse care. The insurance will cover the cost to rent medical equipment (wheelchair, walker, potty..).

[review] => Maybe this place is under new management or something because my family's experience has been positive, unlke other reviewers. My Mom (96) came here after a fall and then broke her arm, she came for rehab but ended up staying in long term care afterwards because her legs were weak and her balance was precarious. The skilled nursing and rehab was very good. Then it ran out because of insurance coverage so she was placed in restorative care which was less frequent. She also needed hygiene and toileting care. But her mind was good. Anyway, we were very pleased with the staff care (CNAs, Nurses, Activities, Kitchen). She was in a private room, almost like when she lived alone at home. My family visited about every day and made friends with staff and other residents. I'm sure that makes a difference. We have since moved out of state and are making that transition, not easy. But the social services and business office have been great to work with. The facility provides a hair salon which is a huge plus. Doctors visit the patients monthly or as needed. Nurses are on the premises. Transportation is available for patients who require outside physicians. The activities staff is awesome. Mom was there for a year. We hated to leave. (2022-2023). Many of the staff are Christian but all faiths and ethnicities are welcome.

[review] => DO NOT place your loved one in this facility. I was moved there from the hospital and it was 3 days later before I was evaluated or even talked to. I had a hip replacement and had to manage to do everything myself. I was in a room with another patient who was so neglected, I started taking care of her needs myself. We constantly rang to get help for a pain med and my roommate to be changed. Someone would come in and instead of helping, said they would send someone in to help but no one ever did. My roommate and her bed were soaked in urine before I finally got one of the competent staff to come in and take care of her needs. I filled out a menu daily but never received what I requested. When I finally went to the dining room to see why I didn't get what I requested, I was yelled at in front of the other residents and staff that they weren't going to accommodate me and they don't always look at the menus handed in. The entire 5 days I was there, there was feces and urine in the bathroom and the room was never properly cleaned and sanitized. I was told by a nurse to contact housekeeping. The tv stopped working and I was told that I would have to contact maintenance. I spoke with other family members of patients and staff members who also had many negative things to say about this facility. The day before I left someone came in to finally check my wound. The stay at this facility was an absolute nightmare and did more harm than good. I shared alot of my concerns with social services and the director who pretended to be concerned. I did speak to a few staff members who were competent and caring and it was expressed how unhappy they were with the way the facility was run. This is the first negative review I have ever written but after this horrid experience found it necessary to do so.

[review] => This review will be a bit long and will include a mix of good and bad things. For background, my dad spent approximately the last six months here, initially for rehab following a severe infection and cognitive issues, and later as a long-term resident until he was recovered enough to move into assisted living elsewhere.
For the good, I'll say that the nursing and rehab staff are hard-working and genuinely care about the patients. To my surprise, my dad made a number of friends among the staff, and that says something about the quality of people there. In particular, some stars on staff or working there include Kamram, Crystal, and Dr Gonzalez. (There are others, but unfortunately their names escape me.) In general, I'm very pleased with the level of attention and care my dad received. The staff goes to great pains to keep the residents stimulated and involved with activities.
On the admin side, Gail is a gem -- she worked with me every step of the way with getting through the Medicaid process, took the time to go through and explain the billing, and kept me well-informed on everything. I couldn't have asked for a better experience!
On the bad side, however, I think the facility is showing signs of strain, possibly due to staffing issues, COVID, or other problems. The laundry service has been, frankly, abysmal. I think by the time my dad was discharged, we left with probably less than half of the clothes I brought to him. Clothing frequently ended up missing or in another resident's closet, even though everything is labeled with each resident's name. Food service quality has been inconsistent, with my dad too frequently getting the wrong meals. Communication from the facility has been inconsistent as well -- some groups (nursing and billing) were great at keeping me informed when I needed to be, while others... not so much. (Like, seriously, I'm POA and health decision-maker for my dad, and when transitioning him to assisted living, I didn't get a *single* phone call from social services, had no input on his discharge date, no communication regarding the discharge procedure, nothing. Fortunately, I worked closely with my ALF and insurance folks and we got through the transfer relatively smoothly.) As an outsider looking in, it seems that internal communication between groups at the facility may be lacking.
Back to the positive -- senior staff holds quarterly meetings directly with residents and their family. This is great! Keep doing this. This gives folks a chance to give direct feedback in a venue where, presumably, it matters. Unfortunately, though, the problems we reported over time to various people either never got resolved or kept reoccurring, which gets you to wondering what the point of it all is.
Another issue we had was with the administration of pain medication to my dad. The meds were to be administered on his request with, I think, a minimum of 12 hours between doses. Limited night and weekend nursing staff ensured that my dad frequently had to wait several hours to get a pill when requested, which throws off the entire schedule. Typically he would need the meds first thing in the morning and before going to bed. Despite requesting this several times to be put on a regular schedule, that never materialized.
To broadly sum it all up, I'll say that I'm grateful to the staff at OCNR. My father is certainly better off now than he was six months ago, and this is directly due to all of the hard work that goes on there. The facility is not perfect -- as I noted, there are several areas which I feel created unnecessary headaches and are deficient enough to knock 2 stars off of a 5 star review; as well, the tendency for reported problems to keep reoccurring is mildly infuriating -- but there's nothing here that screams red flags in my experience. Decide for yourself if I'm being too critical or not critical enough.

Before my Dad left the hospital, he sounded very strong and energetic. He sounded hopeful and happy.
Since he has been at this Rehab Center, his health is slowly declining. He sounds weak and digressing. It worries me that my Father will die there due to their neglect. I have to find a way to get him out of there fast so that he has a chance to survive.
DO NOT SEND YOUR LOVE ONES THERE if you want to see them get well and healthy.

Staff would not answer the phone and take days to return phone calls. Food was so awful that sometimes it could not be eaten. This facility should be shut down.

[review] => My mother was first here after a fall and had hip replacement surgery in May 2020. My mom felt like she never received anything nutritious and her health was degrading and not improving. They didn't check her allergy list and served her eggs one morning for breakfast. They were giving her something daily that by mid-morning to evening, she was so out of it she couldn't think, focus or complete a sentence. We were not able to visit due to Covid19. Make sure you check their medicine list, ensure all allergies are noted if coming from the hospital. Talk to your loved ones daily and at different times of the day. Ask them what they were served for food and if they liked it. Ask them about their room... can they reach their water or any personal items or are they moved away from easy reach. Ask them how long did they wait for staff to come help them to the bathroom. If they seem out of it and can't finish their sentence, something is wrong and they may be overly medicated. I do not recommend this place.

Individual is also with other patients. I did the exercises also in my room, on my own. The first bad thing I noticed was the horrible food. Most meals were not edible. The best food I could get were cereal, banana and yogurt. The orange juice was even subpar. Also, mystery meat!? Hot meals-were not hot. What upset me most was how I saw other patients treated. You have a call bell to ring. Most were not answered promptly. A good 15-20 minutes before someone would check. It is very noisy. The flooring is linoleum and carts are being pushed down the hallways starting 5 AM. During the day staff are noisy talking and yelling to one another. It cannot be a good place to work. There are two Certified Nursing Assistants that are very good--Crystal and Kirsten.

There is also an outdoor area with shelter or sunshine and lots of flowers and greenery.
My Mom is currently recovering after a hospital stay and is very pleased with them.

I noticed in the short period of time we allowed my husband to stay, that the majority of staff are unprofessional and they don't care. She DID NOT wash her hands, so I had to instruct her to do so. No one ever had an appropriate answer when asked a question. No doctor was on site. There was a roaming pet and many patients were left alone in the hallway appearing to be lost and neglected. If this place is rated above standard, I'm afraid to imagine what the nursing/rehabs rated below this psycho house must be like. It was disgusting and disappointing. My husband had three visitors on separate occasions, and each had a horrid opinion . I truly fear for anyone that ends up there.

Short story they never gave my dad the correct medication, he never saw a physician one time in the 4 days he was there before we took him out, nurses never knew he had a picc line so therefore it was never flushed then took it out and that's not against orders. A small dog was roaming in and out of rooms one day when we were there. (We have the pics) I work in the medical field so I am very knowledgeable I realized he had not been given any of the medication the hospital ordered. Staff is very rude and had no clue what was going on. Please if your family means anything to you DON'T TAKE THEM HERE!

[review] => I had surgery on my right foot to remove cancer. I have a graft from my thigh placed over where the cancer was.
I really enjoyed my stay. The nurses, aids and everyone else was very delightful. The physical therapist taught me how to get from my bed to my wheel chair, how to strengthen my left leg and upper body so I can with stand the weight of my body.
I would recommend Orange City Nursing and Rehad.

[review] => While my mother was in Orange City Nursing & Rehab Center, things were fine. I liked the way that she was treated by the people. They were very nice to her. The CNAs and the people from the kitchen seemed to be taking care of her well. I thought the place was very clean. I liked it until I’d seen that they had some lawsuits against them for hiring people that weren’t licensed and finding whoever was on night duty that they weren’t checking on the patients and when the morning duty came, they would find people when they passed away during the night.

[review] => My mother was a resident and received wonderful care from the staff while she was there. The food is very good, and allow families to dine or just sit with their loved one for meals. They have wonderful activities throughout the day for the residents, with music and singers coming in regularly to entertain the residents. We couldn't have been more comfortable or happier with my mother's care and treatment. Theresa is an amazing nurse. Rehab is also "top notch". It's a hard decision to place a loved one in a nursing facility, but this one is exceptional.

[review] => To make it plain and simple, this place is a disgrace when it comes to the care of loved ones. Examples: Loved ones clothing ALL missing, only one person in the laundry staff speaks and reads English, my loved one has gone missing 3 times and resulted in an injury that required stitches, they don't notify you when they should of critical things (such as your loved one being sick for days and you happen to visit them and see them in the bed at 11:30 in the day then all the nursing staff says " I'm really surprised no one called you to tell you"), you give paperwork from the doctor to nursing staff and you hear nothing for 3 mos and when you ask about it no one has a clue what you're talking about, the doctor for the residents never can be seen there and my loved one has been there 2 years and I have never seen or talked with the doctor(called his office 2 years ago to meet and staff said we will return your call and schedule time........that was 2 years ago and I haven't gotten the call yet), Adminstrative staff do "just enough" meaning they manipulate the billings and services to get paid from Medicare and Medicaid, staff is scarce in the facility, they don't tend to residents as they should (not taking them to the bathroom and then yornlives sits in urine or feces for half the day. My thing is, if the shoes were in the other foot, would they want to be treated in such a manner? CLOSE THE PLACE DOWN!!

[review] => My father was in the Rehab section with stage 4 COPD. He called us at 6 in the morning stating that he had his call light on for a half hour asking for help to use the rest room. We called the nursing desk and asked them to check on him. My father called an hour latter and still they had not come to help him, The nurse finally showed up while we were on the phone with him at 8:15. The following day my father was in heart failure which they ignored all day until the ambulance was finally called. He arrived at the hospital with a urinary tract infection and in heart failure. If your loved one is very sick and can't care for themselves do not send them here.

Internet kept dropping and my mom can't speak so her I pad is her only way to keep in touch with family. Finally got her to a room that didn't drop the Internet, only for them to move her back to a room with no internet a day later! Don't bring your loved ones there!

[review] => I went to six nursing homes when I was looking for a facility for my mom. Orange City Nursing was the one that did not smell. The staff seemed the most attentive. Mom needed a fair amount of care but had limited amount of money. They only have semi-private rooms there, so she shares the room with another lady who's very nice and speaks a minimal amount of English. The place is clean, but like most nursing homes they're understaffed. Mom hates the food there because she's on a puree diet. She recently had a stroke, so she cannot chew or swallow solid food.

[review] => My mother was sent to this facility from Fish Memorial in May, 2012 after suffering a stroke. At first the service was great - she was in a private room and got the care and therapy that she needed. Around August, 2012 at the facility managers request we applied for Medicaid since my mom wasn't showing any improvement and would need long term care which Humana didn't cover "“ the manager was very helpful and did all the paperwork. My mom was then moved to a Medicaid room/wing. This part of the facility is not so great. It smells and my mom doesn't get the proper care. She's constantly force to urine/defecate on herself because the staff does not come when called. They even tell her to go on herself and they'll clean her up later. This is not only nasty but can cause a problem with her skin (rashes/sores) since she's blind and confined to the bed/wheel chair all day. My dad came in on one occasion and found my mom crying in bed with her shoes on laying in soiled (urine/feces) clothes. My family and I are devastated and has started searching for a new facility. If you want quality care I don't suggest Orange City Nursing & Rehab.
